An Authorization to Release Information is used when one party grants permission for another party to release information to a third party. This kind of authorization can be used when an individual or business wishes to grant another individual or organization access to confidential information held by a third party.

This document provides consent to release:

• Financial records required by a bank, insurance company, lawyer or company
• Medical records required by a doctor’s office, hospital or insurance company
• Personal information required by a company or employer
